The 22-year-old signed a new five-year deal with the Premier League champions, and has since joined Middlesbrough, who Chelsea sent to the Championship last season, on a season-long loan.
Previous loan spells have seen Baker on loan at Eredivise outfit Vitesse Arnhem for the past three seasons, where he helped them to win the Dutch Cup in April.
In the Chelsea academy, he has won both the FA Youth Cup and U21 Premier League, and made his first-team debut in an FA Cup tie against Derby County.
Baker is also a regular on the international stage, frequently featuring for the England U21 side, and participated at this summer’s European Championships, helping England reach the semi-finals.
He joins a Middlesbrough side who, having suffered relegation last season, will look to bounce straight back to the top-flight.
